Relief Wakes provides goal-based life-skills support for NDIS participants, subject to the participant’s plan, agreed service scope, risk assessment and our capacity. Support is designed with the participant and reviewed over time.

Practising Skills That Matter to You
Life-skills support is planned around the participant’s own goals, preferences and circumstances. Support may include guided practice with household routines, meal preparation, community access, communication or budgeting.
The participant remains in control. We document the agreed activities, progress, risks and any changes needed; independence outcomes cannot be guaranteed.

Maintaining Hygiene & Your Home
Guided practice with agreed personal and household routines, respecting dignity, privacy and choice.
Managing Money
Practical budgeting skills using the participant’s own decisions. Workers do not provide financial advice or control participant funds.
Learning How to Cook & Shop
Guided practice with shopping lists, food purchasing and safe meal preparation, subject to dietary and risk requirements.
Travel & Transport Skills
Building confidence with public transport, planning journeys, and navigating the community independently.
Social & Communication Skills
Developing the social confidence and communication skills needed to connect with others and the wider community.
Medication Management
Medication-related assistance is provided only when specifically authorised, documented and within worker training and competence. We do not diagnose conditions or provide medical advice.

Your NDIS Plan & Goals
Your NDIS plan outlines your goals and the supports you will receive from the NDIS, the community and other government supports to help you achieve those goals.
There are two key sections: "How will I achieve this goal?" and "How will I be supported?" — these help match your goals with the right funded supports.
Funded Support Categories
The goals in your plan will be matched with three funded support types: Core, Capacity-building, and Capital. Some people might have one funded support and others may have all three.

Understanding Plan Reviews
Plan reviews are a normal part of the NDIS. Your plan review will help you to measure your progress against your personal goals, explore new goals, and identify any changes in your life.
When you become a participant in the NDIS, you will receive a plan which funds reasonable and necessary supports that assist you in your daily living. This plan also helps you achieve personal goals such as increasing your independence or learning a new skill.

Explore and set new personal goals building on your previous achievements.
