As life carries forward, it brings its set of ups and downs — and times of transition. These can be when you begin a new job, retire from your current one, or look for a new home. Life Stage Transition can be challenging for everyone, but we at Relief Wakes wish to make them a little less challenging for you.
Workers provide professional, goal-focused assistance within the agreed role and maintain clear boundaries while respecting your choice, privacy and decision-making.

Accessing Life Stage Transition Through Your NDIS Plan

Funding Confirmed First We verify the participant’s current plan, applicable support item and funding arrangement before accepting a service.

Goals & Support Scope The requested assistance must relate to the participant’s goals and fall within the agreed support scope.

Reasonable & Necessary The support must be reasonable and necessary in relation to your disability and the impact it has on your ability to navigate major life transitions independently.

Self, Plan or Agency Managed Relief Wakes supports participants who are self-managed, plan-managed, or agency-managed through the NDIA.

Costs Explained Before Agreement Prices, travel, cancellations and payment arrangements are explained before the participant accepts. Funding approval is not assumed.

What to Expect at Your Plan Review
Plan reviews are a normal part of the NDIS — your opportunity to ensure your Life Stage Transition supports remain aligned with your goals.
Review Your Progress
Measure progress against your personal transition goals, explore new goals, and identify any changes in your life circumstances since your last plan.
Update Your Supports
Provide feedback on what supports are working, request changes to your funding, and ensure your plan reflects your current transition needs and stage of life.
Set New Goals
Set new personal transition goals that build on your previous achievements — whether related to employment, accommodation, or community connection.

How to Access the NDIS
NDIS access requirements and processes can change. Relief Wakes does not determine eligibility. Use the current official NDIS access guidance for your circumstances.
As you prepare to access the NDIS, you might like to work with a family member, friend, carer, an NDIS local area coordinator, early childhood partner, your doctor, or other health professional.
Your doctor or health professional may be asked to complete the NDIS Supporting Evidence Form. Once you're granted access, you'll be contacted to begin your planning conversation. Visit ndis.gov.au or call 1800 800 110.
