Your NDIS Plan

Understanding Your Plan & Community Goals

Your NDIS plan is all about you and your individual goals. Here's how it works for community participation.

🎯 Your Individual Goals

Your NDIS plan outlines your goals and the supports you will receive from the NDIS, the community and other government supports to help you achieve those goals.

If you need any help understanding or using your plan, you will find the myNDIS contact details on the front page of your plan.

📋 Two Key Sections

There are two sections in the plan that are all about achieving your goals in a clearer and more user-friendly format.

1. "How will I achieve this goal" — this will help you with attaining your goals.

2. "How will I be supported?" — this will help you understand who will support you with this goal.

💰 Funded Supports

The goals in your plan will also be matched up with the different funded supports: Core, Capacity-building and Capital. Some people might have one funded support and others may have all three.

For your personalised funded supports, there is a total budget making it easier to monitor and use it flexibly where you can.

🔁 Plan Reviews

Plan reviews are a normal part of the NDIS. Your plan review will help you to measure your progress against your personal goals, explore new goals and identify any changes in your Participation in the Community.

Reviews could be scheduled up to two years apart depending on personal circumstances and goals.

Eligibility & Access

Accessing the NDIS

You may have a few questions about how to access the NDIS. This video will help you answer some of the more common ones.

If you already receive supports from a state or territory government disability programme, you will be contacted when it's time to transition to Participation in the Community supports.

In the meantime, your current supports and services will continue.

⚠️ Eligibility Requirements

To become an NDIS participant, you must have a permanent and significant disability or developmental delay, be of a certain age when you first apply to enter the NDIS, live in Australia, and be an Australian citizen or hold a permanent visa or protected Special Category visa.

As you prepare to access the NDIS, you might like to work with a family member, friend, or carer, an NDIS local area coordinator, early childhood partner, your doctor, or other health professional.

Your doctor or health professional may be asked to provide evidence by completing the NDIS Supporting Evidence Form. Once you're granted access to the NDIS, you'll be contacted to begin your planning conversation.
