Accessing Life Stage Transition Through Your NDIS Plan

Funded Under Capacity Building Life Stage Transition is funded under the Capacity Building — Life Choices budget category in your NDIS plan, focused on planning for life changes.

Transition Goal Required Your plan must include a goal related to a significant life transition — such as leaving school, starting work, moving home, or planning for retirement.

Reasonable & Necessary The support must be reasonable and necessary in relation to your disability and the impact it has on your ability to navigate major life transitions independently.

Self, Plan or Agency Managed Relief Wakes supports participants who are self-managed, plan-managed, or agency-managed through the NDIA.

No Out-of-Pocket Cost When funded through your NDIS plan there is no out-of-pocket cost to you. We claim directly through the NDIS portal on your behalf.

What to Expect at Your Plan Review
Plan reviews are a normal part of the NDIS — your opportunity to ensure your Life Stage Transition supports remain aligned with your goals.
Review Your Progress
Measure progress against your personal transition goals, explore new goals, and identify any changes in your life circumstances since your last plan.
Update Your Supports
Provide feedback on what supports are working, request changes to your funding, and ensure your plan reflects your current transition needs and stage of life.
Set New Goals
Set new personal transition goals that build on your previous achievements — whether related to employment, accommodation, or community connection.
Review Timing
Plan reviews may be scheduled up to two years apart. Your NDIS representative will ensure you're comfortable with the timing and given adequate time to achieve your goals.
Children (0–6 Years)
For children up to six years of age, most plan reviews are conducted by your Early Childhood Early Intervention Partner in collaboration with your family.
Ages 7–65 Years
For people aged 7 to 65, plan reviews are conducted by a Local Area Coordinator or an NDIS planner, depending on your situation and location.
How to Access the NDIS
To become an NDIS participant, you must have a permanent and significant disability or developmental delay, be of a certain age when you first apply, live in Australia, and be an Australian citizen or hold a permanent visa or protected Special Category visa.
As you prepare to access the NDIS, you might like to work with a family member, friend, carer, an NDIS local area coordinator, early childhood partner, your doctor, or other health professional.
Your doctor or health professional may be asked to complete the NDIS Supporting Evidence Form. Once you're granted access, you'll be contacted to begin your planning conversation. Visit ndis.gov.au or call 1800 800 110.
🌟 Already have an NDIS plan?
If your plan includes Life Stage Transition or Capacity Building — Life Choices funding, you can start with Relief Wakes right away.
If you already receive supports from a state or territory government disability programme, you will be contacted when it's time to transition to the NDIS. In the meantime, your current supports and services will continue.
